COMPREHENSIVE CARE PLANS

In 2014 we began a pilot program to improve care co-ordination among primary care providers and specialists.  Our goal was to reduce medical errors, improve quality of care, and reduce hospital admissions.  

Medical care has become more and  more fragmented.  Patients were being seen by urgent care providers, nurse practitioners, hospitalists, and specialists.  Many of these providers are in differt locations and different heatlhcare networks.
